The Community Aging Referral Program (CARP) allows individuals with higher assets to purchase these same services with a referral from Senior Services.
CARP is a new referral system for low asset Comprehensive Care Coordination participants who do not qualify for Community Care Program (CCP) services due to slightly higher income, but who are in need of services. Aging and Provider have mutual interests in preventing premature institutionalization for older persons and, as a result, agree to the following to implement the CARP.
Aging will direct Care Coordination Units (CCUs) to refer low asset CCP applicants (those with assets over $17,500 but below $35,000) who do not qualify for CCP to qualified CARP providers.
CCUs will refer these low asset applicants to qualified CARP providers, who must agree to provide private pay services to low asset applicants at cost no higher than the current State rate for the comparable CCP service.
Provider must work with the CARP participant/authorized representative to develop an appropriate service plan.
Participant solely responsible for payment of services provided; payment schedules and collection are issues to be resolved between the Provider and the CARP participant.
